Forms to Complete Online
In order to cut down on the number of forms turned in, we are asking all parents to please log into PowerSchool (https://stm.powerschool.com/public) on a computer or tablet, not on the app. On the left column, there is a little icon called Forms. Please click on that. Please fill out Forms A, B, D (if needed), E, Y, and Z. This will prevent us from having to track down forms, and will also allow us to search more quickly on PowerSchool if we have permission to give your student any treatments or what hospitals you prefer or if they have permission to be in any of our photos we use for promotional materials. Please let us know if you have any specific questions and we will work to get those answered for you.
Any student or staff member driving to school must fill out this vehicle registration form.

A Registration Form must be completed for each student.
According to the IHSA, any student wishing to compete in a sport will need to complete and turn in the forms found on the front page of the school website. Click on the yellow button in the middle of the page called Athletic Forms.
- All new students will need to turn in a physical, immunization record, and dental record. Seniors must turn in their updated immunization record to keep on file in the office. Students coming from outside of Illinois must turn in an eye exam record, as well. These have all been sent from Mrs. Oliveira but can also be found on our school website HERE.
